In consideration of your/your child’s participation in this Program and all related activities, you acknowledge, are aware of, appreciate and accept the inherent physical risks and other possible RISKS, DANGERS, AND HAZARDS associated with you/your child being a participant in this Program. You also acknowledge, understand, covenant and agree as follows:
- My/my child‘s participation in this Program is purely voluntary
- The church leaders are volunteers and not formally trained
- I understand it is my responsibility in making sure that my child arrives and is picked up on time for events, activities and services.
